Responsibilities Serve as the primary point of contact for patients after their intake visit Help patients understand and act on their care plan, including scheduling appointments and finding in‑network providers Resolve navigation friction regarding portals, referrals, appointment logistics, and provider communications Identify practical and social‑needs support such as food, housing, and transportation and connect patients to resources Coordinate with clinical team members while providing non‑clinical support only Document every patient interaction and support activity in the platform accurately and on time Maintain confidentiality and comply with HIPAA and privacy standards Requirements Active, unrestricted LPN/LVN license, Community Health Worker (CHW) certification, or equivalent patient‑navigation experience 2+ years of patient‑ or community‑facing support experience Working knowledge of the U.S. healthcare system and Medicare Ability to work independently and manage a caseload Reliable computer, internet, and a quiet space for patient calls Preferred Qualifications Experience supporting older adults or patients with complex or serious illness Familiarity with Medicaid and other public programs Bilingual skills in Spanish, Mandarin, or other languages common among Medicare patients #J-18808-Ljbffr
